Oasis Family Dentistry Warranty Information


Oasis Family Dentistry makes every effort to  provide you with the highest quality dental appliances. We use quality materials, well established labs, as well as the best care and diligence. We are confident in the quality of our work and support it with a warranty that very few dental offices offer. We stand behind our work and want you to have a beautiful, long lasting appliance. 


APPLIANCE WARRANTY


This warranty covers defects in materials and workmanship, including delivery, with the exceptions stated below as well as lost or damaged appliances.


This warranty does not cover any discoloration or failure of the appliance caused by cavities/decay, smoking or consumption of staining liquids, foods, abuse, neglect, damage, or normal wear and tear. Should the appliance require repair or replacement, additional treatments associated with the repair are not covered.


All removable appliances such as mouth guards, essex retainers, and any other appliance falling under this category is covered. If lost or damaged within one year of the original placement, a new appliance will be made at 50% discount of the current year's price. If within 2 years of the original placement, a new appliance will be made at 25% off the current year's price. And the third year and beyond, the appliance will be at the full current year's price. You must maintain your regular recommended cleaning appointments from the date of delivery for the warranty to remain active. Please bring your appliance with you to all cleaning and recare appointments so we may examine and clean the appliance. Cleanings are recommended as follows: Regular healthy patients- 2 calendar year cleanings. Periodontially compromised patients- 3-4 cleanings per year.


Smoking, excessive clenching, ice chewing, failure to maintain cleaning appointments and recommended radiographs, refusal of treatment for diagnosed bruxism will void this warranty. Direct repairs or alterations to the appliance by anyone other than Oasis Family Dentistry employees will also void the warranty. 



RESTORATIVE WARRANTY 


This warranty covers only defects in materials and workmanship, including installation, with the exceptions stated below.


This warranty does not cover any discoloration or failure of a crown/filling caused by cavities/decay, smoking, or consumption of staining liquids or foods. Abuse, neglect, damage, and normal wear and tear are also not covered. Should the crown or filling require repair or replacement, additional treatments associated with the procedure are not covered.

Crowns, bridges, inlays, and onlays are covered for 5 years from the date the restoration is delivered. Composite restorations are warrantied for 1 year from the completed procedure. You must remain a patient of Oasis Family Dentistry for the entirety of the warranty period following placement of the restoration. You must maintain a minimum number of recommended cleaning appointments every calendar year from the date of delivery. Cleanings are recommended as follows: Regular healthy patients- 2 calendar year cleanings. Periodontially compromised patients- 3-4 cleanings per year as recommended.


Smoking, excessive clenching, ice chewing, refusal of treatment for diagnosed bruxism, failure to maintain cleaning appointments and recommended radiographs will void this warranty. Failure to complete, in our office, all treatment in the area of the mouth where the crown/bridge is located within 3 months of being diagnosed and treatment planned. Direct treatments on the restoration by any persons other than Oasis Family Dentistry employees will also void this warranty.


Oasis Family Dentistry will repair a restoration that proves to be defective in materials or workmanship. In the event the repair is not possible, Oasis Family Dentistry will replace the crown or restoration with a new crown or restoration of similar material and price. 



DENTURE/PARTIAL WARRANTY


This warranty does not cover any discoloration or failure of the appliance caused by cavities/decay, smoking or consumption of staining liquids, foods, abuse, neglect, damage, or normal wear and tear. Should the appliance require repair or replacement, additional treatments associated with the repair are not covered. Healing dentures are not covered under this warranty as they are only used during the first year of healing. However, 1 chairside reline is included for healing dentures and post-operative adjustments are unlimited for the first year. 


Removable and /or permanent appliances such as dentures, partial dentures, flippers, and any other appliance in this category are covered.  If lost or damaged within one year of the original placement, a new appliance will be made at 50% discount of the current year's price. If within 2 years of the original placement, a new appliance will be made at 25% off the current year's price. And the third year and beyond, the appliance will be at the full current year's price. You must maintain your regular recommended cleaning appointments from the date of delivery for the warranty to remain active.
